Q3: Our imam led us in prayer, and in the second rak'ah of the Zuhr prayer, when sitting for the first tashahhud, he prostrated once, meaning: he forgot and did not prostrate the second time, and when he completed the prayer, one of the ma'mum mentioned his forgetfulness and told him to repeat a rak'ah and perform sujud al-sahw. How valid is that? Does he perform sujud al-sahw, or does he perform a rak'ah and then prostrate for forgetfulness?
A3: If the situation is as you described, the rak'ah in which he left the prostration out of forgetfulness is invalidated, and the third becomes the second and the fourth becomes the third. He then performs a rak'ah to be the fourth, and then prostrates for forgetfulness.
