Question:
What is the ruling on forgetfulness in prayer?

Answer:
Forgetfulness in prayer occurs among people, even among the prophets, but the Prophet (peace and blessings be upon him) explained the ruling on forgetfulness. It has categories and types. If the one praying forgets the first tashahhud, or some of the takbirs other than the first takbir, other than the takbir of ihram, the takbir for ruku' or sujud, or something similar, he prostrates for forgetfulness, two prostrations, before he says the taslim, and praise be to Allah, if he is an imam or praying alone. As for the ma'mum, he does not prostrate; rather, he follows his imam.
As for if the forgetfulness is doubt, such that a man or woman, whether he or she is an imam or praying alone, doubts whether he or she prayed two or three rak'ahs of Dhuhr, 'Asr, Maghrib, or 'Isha, he or she makes it two, acting on certainty, meaning: the lesser number. Then he or she says takbir and prostrates for forgetfulness, two prostrations, before saying the taslim. If he or she doubts in Dhuhr, 'Asr, or 'Isha whether he or she prayed three or four, and is in doubt, he or she makes it three, exercising caution by building on the lesser number, then he or she performs the fourth. When he or she is finished, he or she prostrates two prostrations before saying the taslim.
This is what is prescribed for him. And if he forgets the first tashahhud and stands up to the third without the first tashahhud, forgetting it, while he is an imam or praying alone, he prostrates for forgetfulness after finishing, before saying the taslim. And if he is a ma'mum, there is no prostration required of him; rather, he follows his imam. Yes.
