Did he not become forgetful in his prayer? He became forgetful once and prayed the midday prayer with five rak'ahs. He became forgetful once and concluded the prayer with two rak'ahs of the midday or afternoon prayer. He became forgetful once and stood up without performing the first tashahhud.
