What is known from the hadiths is that the Prophet (peace and blessings be upon him), when he raised his head from the bowing, would say:
Allah hears whoever praises himسمع الله لمن حمده
Muwatta Malik 3:17
, then after that he would say:
Our Lord, to You belongs all praiseربنا ولك الحمد
, and sometimes he would say:
Our Lord, to You is the praiseربنا لك الحمد
, and sometimes he would say:
O Allah! our Lord, unto Thee be praiseاللهم ربنا لك الحمد
Sahih Muslim 476b
, and sometimes he would say:
Allahumma Rabbana wa lakal-hamd (O Allah, our Lord and to You be the praise)اللهم ربنا ولك الحمد

O Master of praise and glory, You are worthy of what the servant says, and we are all Your servants. There is no one who can prevent what You have given, and there is no one who can give what You have withheld, and the wealth of the wealthy does not benefit him in the face of You.أهل الثناء والمجد، أحقّ ما قال العبدُ، وكلنا لك عبدٌ، لا مانع لما أعطيتَ، ولا مُعطي لما منعتَ، ولا ينفع ذا الجدِّ منك الجدّ

This is prescribed for the believing man and the believing woman, and for the imam, the one praying alone, and the one praying behind the imam; it is prescribed for all of them. However, the imam and the one praying alone begin with
Allah hears whoever praises himسمع الله لمن حمده

As for the word "wa al-shukr" (and gratitude), I do not know of any basis for it. However, it means the same as al-hamd (praise); because al-hamd means gratitude and praise. So if al-hamd is mentioned, al-shukr is included in it. Al-shukr is praising Allah with words and deeds, so there is no need for it; it is included in his saying:
Our Lord, to You belongs all praiseربنا ولك الحمد
. If a person says it, it does not harm his prayer, and there is no blame on him for it. But leaving it out is more appropriate; because it was not transmitted. Otherwise, it is included in the meaning of al-hamd. But the believer does not bring something from his own mind. Leaving it out is more appropriate, so he should restrict himself to saying:
Our Lord, to You belongs all praiseربنا ولك الحمد
, and that is sufficient. He does not need to add "wa al-shukr"; because it was not transmitted in the texts, as far as we know. Even though it is included in the meaning of al-hamd, the believer should stick to the texts, and acts of worship are based on divine legislation. So it is better for the believer to say:
Our Lord, to You belongs all praiseربنا ولك الحمد
only, without adding "wa al-shukr" when he rises from the bowing. Some of the common people say this word, but it does not harm their prayer. However, they should be guided and taught that it is better to leave it out; because it was not transmitted.
