Question:
By virtue of our work as doctors, we are sometimes compelled to examine some women, and certain thoughts come to our minds, but we strive against them. Do we bear any sin for that?

Answer:
With striving, no. He strives and he guards himself. The Prophet (peace and blessings be upon him) said:
Indeed, if a servant intends to do an evil deed but does not do it, nothing is recorded for him. If he intends it and leaves it for the sake of Allah, Allah records it for him as a good deed.إنَّ العبد إذا همَّ بالسيئة فلم يعملها لم يُكتب عليه شيءٌ، فإن همَّ بها وتركها من أجل الله كتبها الله له حسنةً
, so he is not held accountable for it unless he acts on it. And the Messenger (peace and blessings be upon him) said:
Allah, the Most High, has forgiven my followers what they contemplate on within themselves (i.e. evil thoughts) as long as they do not act upon or speak about them.إنَّ الله تجاوز عن أمتي ما حدثتْ به أنفسها، ما لم تعمل، أو تكلم
Bulugh al-Maram 8:133
. And if he fears for himself and his Shaytan, he should not treat the woman; he should make an excuse and refer her to someone else, for his religion is more binding upon him.
