The imam added a prostration, meaning he prostrated three times in one rak'ah, so what is the ruling?

Machine translationRead the Arabic original
Q2: A man led us in prayer as imam and added a prostration, meaning he prostrated three times in one single rak'ah, so what is the ruling regarding this rak'ah?
A2: If the imam adds a prostration in one of the rak'ahs intentionally, his prayer is invalid. If he adds it out of forgetfulness, he performs the prostration for forgetfulness and his prayer is valid.
And success is from Allah, and may Allah send prayers and blessings upon our Prophet Muhammad, his family, and his Companions.
