Q: I am a person whom Allah has afflicted with a disease in the urinary tract, and I have done what I can to seek treatment. All the doctors were baffled by the nature and cause of this disease. By the facilitation of Allah the Exalted, I presented myself in the preceding month of Ramadan to a specialist consultant in these diseases. After a precise examination, the following was revealed: the presence of tissues lining the inner wall of the penis, whose function is to secrete natural fluids in a normal person. However, my condition showed that these tissues have become enlarged, so they secrete this fluid in large quantities. This fluid resembles madhi in color and nature, differing from it only in that it has no foul odor and is not accompanied by desire. This puts me in a difficult situation regarding the issue of ritual purity. Before this examination, I assumed it was madhi, but the doctor clarified to me that the tissues I mentioned earlier are responsible for secreting this fluid, and that the rest of the organs and systems are 100% healthy. The question is: A - Is this fluid pure or ritually impure?
B - If it is ritually impure, am I required to purify my clothes and thighs, and am I required to perform ritual purity for every prayer? C - Should I use a protective device on the penis to prevent the spread of this fluid? Knowing that this involves extreme hardship.
A: If the situation is as you described, and that a fluid resembling madhi in color and nature emerges from your penis, but without a foul odor, continuously and without desire, then its ruling is the ruling of incontinence (salsal al-bawl). It is obligatory to perform istinja' and wudu' from it for every prayer, and to prevent its spread on the trousers and thighs by using a specific protective device according to your ability. This is based on the saying of Allah the Exalted:[At-Taghabun 64:16] . We ask Allah for your healing and reward for what has befallen you.
