Q: I have a sister who underwent several surgeries in the uterine area, including abdominal opening surgeries and cleaning procedures, which resulted in her menstrual cycle becoming continuous, to the point where it now comes to her two to three times a day and night, and sometimes it continues in the form of simple drops, though it does not stop. It is known that before the surgeries, her cycle was
regular, and after the surgeries it became irregular, and when the surgeries increased, it became continuous. She is currently taking a medical prescription at a rate of four times a day and night. The question: 1 - She did not fast Ramadan last year, and to date she has not been able to make up the fasts, so how does she make up the past year? And how does she fast this year, given that the doctor advises against stopping the medical prescription, fearing complications, may Allah prevent it? And Allah knows best. 2 - She is currently praying some obligatory prayers and leaving others, so what is the ruling on her prayer? And how should she pray (when)? 3 - Knowing how she should perform Hajj and 'Umrah if she remains in this state. The intent is to know the Shari'ah ruling regarding fasting, Hajj, 'Umrah, and prayer. How does she perform all of the above if she remains in this state? Please inform us, may Allah reward you with the best reward, regarding Islam and the Muslims, and may Allah heal everyone from every ailment.

A: This woman is in the category of a woman with habitual irregular bleeding; because she had a regular cycle before undergoing these procedures, she knows the time of her previous cycle and her purity from it. Accordingly, she observes her previous cycle, so she leaves off the ritual prayer and fasting during it. When the number of days of her previous cycle has passed, she performs the full ritual bath and binds her private parts with cotton or the like to prevent the blood from coming out, as much as possible.
She prays and fasts if she is able to fast, and she makes up the fasting of the days of her cycle that she observed. She has the ruling of women in a state of purity regarding the permissibility of her husband having intercourse with her, even if blood or drops of blood are coming out; because what she sees after the days of her cycle is irregular bleeding. However, she performs the ablution for the time of every prayer if anything comes out of her after the previous ablution. If, after undergoing the aforementioned procedures, fasting is difficult for her and she is harmed by not taking her medication during the day, and complications arise due to its omission, or her recovery is delayed as the treating doctor told her, then she is in the category of a sick person, and it is permissible for her to break her fast during fasting. If this condition is generally expected to be cured, she waits until she is cured, if Allah wills, and then fasts the number of days she did not fast in Ramadan. If this condition is not generally expected to be cured, and it continues with her, and the doctors have stated this, then she feeds one poor person for every day she did not fast. The amount of feeding is: half a sa' (a dry measure of about 3 kg) of wheat, dates, rice, or the like of what the people of the country eat as their staple food. Its weight is approximately one and a half kilograms. There is no blame on her for breaking her fast, and she does not have to make up the fast in this case; because of the saying of Allah the Exalted:
And upon those who are able [to fast, but with hardship] - a ransom [as substitute] of feeding a poor personوَعَلَى الَّذِينَ يُطِيقُونَهُ فِدْيَةٌ طَعَامُ مِسْكِينٍ
[Al-Baqarah 2:184]

Allah does not charge a soul except [with that within] its capacityلاَ يُكَلِّفُ اللَّهُ نَفْسًا إِلا وُسْعَهَا
[Al-Baqarah 2:286]

and has not placed upon you in the religion any difficultyوَمَا جَعَلَ عَلَيْكُمْ فِي الدِّينِ مِنْ حَرَجٍ
[Al-Hajj 22:78]

As for her Hajj and 'Umrah (the lesser pilgrimage), if the days of her cycle in the month that she knows from when she was regular coincide with the days of her Hajj and 'Umrah, it is recommended for her to perform the full ritual bath and enter the consecrated state of pilgrimage for Hajj or 'Umrah; because the Prophet (peace and blessings be upon him) (orderedAishathat she perform the full ritual bath to enter the state of consecration for Hajj while she is menstruating), and it is authentic that he (peace and blessings be upon him) (orderedAsma bint Umayyiswho was in post-natal bleeding to perform the full ritual bath). She does what the pilgrim does, except that she does not circumambulate the House during her known menstrual days. If her menstrual days end, or if her menstrual days do not coincide with the days of Hajj and 'Umrah while she is in the state of consecration for them, she is in the ruling of a woman with irregular non-menstrual bleeding; she binds her private parts with cotton or the like so as not to defile the mosque, and she circumambulates, and there is no blame on her, and she completes the rest of the rituals of her Hajj and 'Umrah. And success is from Allah, and may Allah send prayers and blessings upon our Prophet Muhammad and his family and his Companions.
