Question:
When is the prostration of forgetfulness obligatory, and how is it performed?

Answer:
If one leaves out an obligatory act, or does something forbidden out of forgetfulness, the prostration of forgetfulness becomes obligatory. However, if one leaves out a recommended act, the prostration is not required. For example, if one forgets the opening supplication, or forgets the supplication in prostration, or something similar, or forgets to raise the hands, the prostration is not required. The prostration is only required if one leaves out an obligatory act out of forgetfulness, such as leaving out the first tashahhud and standing up, or forgetting a prostration and then performing the following rak'ah and completing the prayer, or adding an extra rak'ah.
The point is: if one leaves out an obligatory act out of forgetfulness, he must perform the prostration. For example, leaving out "Subhan Rabbi al-A'la" in prostration, "Subhan Rabbi al-Azim" in bowing, or "Rabbana wa laka al-hamd" when rising from bowing, or the takbirs when going into prostration and rising from it. If he forgets any of these, it is better to prostrate for it at the end of the prayer before saying the taslim. The same applies if he leaves out the first tashahhud.
As for if one leaves out a pillar, such as the prostration of the prayer, or such as bowing, and does not remember it until later, the rak'ah that follows takes the place of the rak'ah in which he left it out. The rak'ah in which he left out the prostration and bowing is lost, and the rak'ah that follows takes its place. Then he performs an extra rak'ah. When he completes it, he prostrates for forgetfulness, then says the taslim. This is also obligatory, because a person may forget bowing or prostration and not be reminded, then he remembers or is reminded later. He then performs a complete rak'ah in place of the rak'ah in which he left out the prostration or bowing.
However, if he is reminded immediately that he did not bow while he is going down into prostration, he returns, stands up, then bows. Or if he is reminded that he did not perform the second prostration, he prostrates immediately. If he has already stood up and is reminded, he sits and says: "O Allah, forgive me, O Allah, forgive me," then he performs the second prostration, and prostrates for forgetfulness at the end of the prayer.
