Question:
Is supplication after the tashahhud in the second rak'ah, or in the four-rak'ah prayer, considered an error?

Answer:
Supplication in the first tashahhud is not prescribed. If one does it, there is no error on his part, and no prostration for forgetfulness is required. However, if he does prostrate, there is no harm, because he did it in a place where it is not due. As for supplication in the final tashahhud, it is prescribed.
Supplication in the final tashahhud before the taslim is prescribed and commanded. He should supplicate with good and well-known supplications; the well-known supplications are better. Examples include seeking refuge in Allah from the punishment of the Hellfire, from the punishment of the grave, from the trial of life and death, and from the trial of the Dajjal (the false messiah), as well as asking for forgiveness and pardon, and saying:
O Allah, help me in remembering You, in giving You thanks, and worshipping You well.اللهم أعني على ذكرك وشكرك وحسن عبادتك
