May Allah reward you with good.
A: It is permissible for you to shake the hand of your grandfather’s wife on the father’s side; because you are one of her mahram (relatives one may not marry). And success is from Allah, and may Allah send prayers and blessings upon our Prophet Muhammad, his family, and his Companions.
