Question: A man has been suffering from diabetes for three years. He used to fast the month of Ramadan, but with great difficulty. Is it permissible for him to break his fast this year, and what is required of him if he does? With that illness, he constantly feels hunger and thirst, even if the weather is mild?

Answer: Fasting the month of Ramadan is one of the pillars of Islam. Allah the Exalted said:
O you who have believed, decreed upon you is fasting as it was decreed upon those before you that you may become righteous -يَاأَيُّهَا الَّذِينَ آمَنُوا كُتِبَ عَلَيْكُمُ الصِّيَامُ كَمَا كُتِبَ عَلَى الَّذِينَ مِنْ قَبْلِكُمْ لَعَلَّكُمْ تَتَّقُونَ
[al-Baqarah: 183],until His saying:
So whoever sights [the new moon of] the month, let him fast itفَمَنْ شَهِدَ مِنْكُمُ الشَّهْرَ فَلْيَصُمْهُ
[al-Baqarah: 185].

So whoever among you is ill or on a journey [during them] - then an equal number of days [are to be made up]فَمَنْ كَانَ مِنْكُمْ مَرِيضًا أَوْ عَلَى سَفَرٍ فَعِدَّةٌ مِنْ أَيَّامٍ أُخَرَ
[al-Baqarah: 184]. So the sick person breaks his fast and makes up the days he missed from other days, if that is feasible. You are sick, so you may break your fast if fasting is difficult for you, or if fasting increases the illness, or if it aggravates the illness. In that case, you break your fast in accordance with the dispensation of Allah (Glorified and Exalted is He).
Then, if you are able to make up the fasts in the future, it is obligatory for you to make up the days you missed, because of the saying of the Exalted:
an equal number of days [are to be made up]فَعِدَّةٌ مِنْ أَيَّامٍ أُخَرَ
[al-Baqarah: 184]. And if you are unable to make up the fasts because the illness is chronic and an illness from which recovery is not expected, then it is incumbent upon you to feed one poor person for each day. This is approximately one and a half kilograms of food; you give out one and a half kilograms of food for each day. This is if you are unable to make up the fasts because the illness continues with you and is chronic. The person with a chronic illness and the very old, decrepit elder break their fasts and feed, and they do not have to make up the fasts. However, if it is within your ability, or if you can expect that this illness may be removed, or may lessen, or if there is a time in the year, for example, when it lessens for you and you are able to fast, then you postpone the fasting until that time. But if none of that is the case, then you feed one poor person for each day, and that is sufficient for you.because of His saying, the Exalted:
And upon those who are able [to fast, but with hardship] - a ransom [as substitute] of feeding a poor personوَعَلَى الَّذِينَ يُطِيقُونَهُ فِدْيَةٌ طَعَامُ مِسْكِينٍ
[al-Baqarah: 184]. Among them is the sick person from whose illness recovery is not expected.
