I see that if dire necessity calls for it, and you have no choice but to do so, then fear Allah as much as you are able. Do not converse with them much, except in what dire necessity requires, and lower your gaze from them as much as possible. We ask Allah the Exalted to bring the day when men will be independent in their sciences and women in theirs.
As for the issue of seclusion; it is not permissible at all. But how can he be in seclusion with her when the patient is with them?! Except if the patient is not aware! The questioner: At some times, when I come, I laugh or greet.

Do not laugh. Do you laugh at a woman who is a stranger to you? Never. But do not show her a frowning face or an overly cheerful face. Be natural out of necessity. If she sees that you will report on her if she neglects what is obligatory upon her, she will not neglect the obligation, especially a stranger who knows that if she neglects the obligation, it will lead to exposure.
