Machine translationRead the Arabic original
The questioner : The honorable Shaykh, this questioner asks: What is the correct formula for sending blessings upon the Prophet (peace and blessings be upon him)?

The Shaykh : Praise be to Allah, Lord of the worlds, and may Allah send blessings and peace upon our Prophet Muhammad, and upon his family and his Companions, and upon those who follow them with excellence until the Day of Judgment:
The correct formula for sending blessings upon the Prophet (peace and blessings be upon him and his family) is what he taught his Ummah, where they said: “O Messenger of Allah! Teach us how to greet you, and how should we send blessings upon you?” He said:
O God, bless Muhammad and Muhammad’s family as Thou didst bless Abraham and Abraham’s family. Thou art indeed praiseworthy and glorious. O God, grant favours to Muhammad and Muhammad’s family as Thou didst grant favours to Abraham and Abraham’s family. Thou art indeed praiseworthy and glorious.
Mishkat al-Masabih 919
This is the transmitted formula.
Other formulas have been transmitted, and whoever sends blessings upon him with them is in a good state, because this is an area in which the Sunnah has varied.
As for what is found in some books of prayers built on rhyming prose and descriptions, which may include descriptions that are not valid except for the Lord of the worlds, beware of them, and flee from them as you would flee from a lion. Let not the rhyming prose in them deceive you, which may make the eye weep and soften the heart. Stick to the authentic and the fundamentals, and leave this which was composed without guidance or authority.
