You ask and say: What is the form of prostration of gratitude? May Allah reward you with good.

Answer:
The prostration of gratitude is like the prostration of prayer, like the prostration for forgetfulness, and like the prostration of recitation: it is one prostration. In it, one says: "Subhana Rabbi al-A'la" (Glory be to my Lord, the Most High), "Subhana Rabbi al-A'la," praises Allah, extols Him, and thanks Him for what He has bestowed upon him, such as health, a child, victory for Islam, a conquest for the Muslims, or something similar that brings him joy or brings joy to the Muslims. The Prophet (peace and blessings be upon him) prostrated in gratitude, and so did al-Siddiq (may Allah be pleased with him) when the news of the conquest of al-Yamamah and the killing of Musaylimah the Liar reached him; he prostrated to Allah in gratitude.
And the correct view is that it is permissible even if the one prostrating is not in a state of ritual purity. When the glad tidings reach him, he prostrates, even if he is not in a state of ritual purity.
And so it is with the prostration of recitation, which is of the same kind as the prostration of gratitude: one prostration when reciting the verses that contain prostration, even if one is not in a state of ritual purity, according to the stronger of the two opinions of the scholars.
In it, one says what is said in the prostration of prayer, the prostration of recitation, the prostration of gratitude, and the prostration for forgetfulness; in all of them, one says what is said in the prostration of prayer: "Subhana Rabbi al-A'la, Subhana Rabbi al-A'la... Subhana Allah Rabbana wa bihamdika, Allahumma ighfir li... Subbuhan Quddus Rabb al-Mala'ikah wa al-Ruh" (Glory be to You, O Allah, our Lord, and with Your praise, O Allah, forgive me... And one also says in the prostration:
O Allah, it is to Thee that I prostrate myself and it is in Thee that I affirm my faith, and I submit to Thee. My face is submitted before One Who created it, and shaped it, and opened his faculties of hearing and seeing. Blessed is Allah, the best of Creatorsاللهم لك سجدت، وبك آمنت، ولك أسلمت، سجد وجهي للذي خلقه، وصوره، وشق سمعه، وبصره بحوله وقوته، تبارك الله أحسن الخالقين
