Your Eminence, Shaykh Abdul Aziz ibn Baz (may Allah guide you to success), Allah has afflicted me with kidney failure, and I had a kidney transplant. The doctors advised me not to fast during Ramadan. I ask Allah, and then you, for guidance and correctness. Do I have to pay an expiation or not? I am very grateful to you.

Answer:
This depends on you: If you feel what the doctors said, that fasting harms you, there is no harm in not fasting until the harm is removed; because Allah says:
and whoever is ill or on a journey - then an equal number of other daysوَمَنْ كَانَ مَرِيضًا أَوْ عَلَى سَفَرٍ فَعِدَّةٌ مِنْ أَيَّامٍ أُخَرَ
[Surat al-Baqarah: 185], and you are sick in the kidney—the correct term is kidney—if the knowledgeable doctors advise you not to fast, do not fast. But if you find in yourself what indicates that it does not harm you, and you have tried this matter and it does not harm you, then fast, and praise be to Allah; because if you know that this does not harm you, you are not sick, so you must fast. And as long as you feel sick when you fast, as the doctors said, do not fast until you are cured.
And if the doctors determine that it harms you permanently, then you must feed one poor person for every day, like the old, decrepit man and the old, decrepit woman who are unable to fast; they feed one poor person for every day, and they do not have to fast.
If the specialist doctor, and it is better to have two doctors or more, to be cautious for yourself, if they determine that fasting is not possible, and that it absolutely harms you; then you feed. If they say that this kidney disease will continue—if you had a kidney transplanted—and if they are hesitant or they disagree, you must fast. If you feel sick, break your fast, and if you feel healthy, continue fasting. You are the doctor of yourself; you know what harms you and what benefits you.
