As far as I recall, some of them appeared in the speech of the Prophet (peace and blessings be upon him), some were taken from the chapter itself, and some from the Companions (may Allah be pleased with them) and their best, as Ibn Mas'ud (may Allah be pleased with him) said when he stoned the pillar from the bottom of the valley: "This is the place where the chapter of al-Baqarah was revealed to him," and as Abu Umamah said: "The bearers of the Qur'an will be called on the Day of Resurrection, and they are those who recited it and acted upon it, with the chapters of al-Baqarah and al-Imran leading them," and some are taken from the meanings of the chapter.
So, as far as I know, there is no explicit text for every chapter stating that the Prophet (peace and blessings be upon him) named it. Rather, some were taken from his naming (peace and blessings be upon him), some from the naming of the Companions, and some from the chapter itself.
As for their arrangement in the mushaf: starting with al-Fatihah, then al-Baqarah, then al-Imran, ... to the end, this arrangement is from the independent legal reasoning (ijtihad) of the Companions (may Allah be pleased with them).
