Question:
He says: I was afflicted with an illness in a past year, more than four years ago, during the month of Ramadan, and I did not fast that month, nor have I made it up until now. So what is obligatory for me to do?
May Allah reward you with good.

Answer:
If you delayed it without an excuse, you must repent. You must repent to Allah the Exalted, feel remorse for the negligence that has passed on your part, and make a sincere resolve not to return to such a thing. You must fast, and you must also feed a poor person for each day if you are able, because you were negligent.
A group of the Companions issued a ruling that one who was negligent must feed a poor person in addition to making up the fast. So you must make up the days, and you must feed a poor person for each day: half a sa' of dates, or rice, or wheat, which is one and a half kilograms for each day. Along with that, you must repent to Allah for your negligence and delay, because it is obligatory to hasten to make up the fast before the new Ramadan, and you have delayed a great deal. So you must repent to Allah for that. You must make up the days, whether consecutively or separately, there is no harm in that. And along with that, if you are able, you must feed a poor person for each day. You gather the food and give it to some of the poor people according to the number of days, half a sa' for each day, meaning one and a half kilograms for each day. You gather it and give it to some of the poor people, even if it is just one poor person.
As for if you are poor and cannot afford the food, then you only fast along with repentance, and that is sufficient. Because Allah the Exalted says:
Allah does not charge a soul except [with that within] its capacityلا يُكَلِّفُ اللَّهُ نَفْسًا إِلَّا وُسْعَهَا
[al-Baqarah:286],
So fear Allah as much as you are ableفَاتَّقُوا اللَّهَ مَا اسْتَطَعْتُمْ
[al-Taghabun:16]. Yes.
