Q: I have a woman who underwent surgery before the month of Ramadan, and Allah did not write that she would fast before the surgery. The surgery was as follows: the complete removal of one kidney and the extraction of a stone from the second kidney. The doctors advised her not to fast for the rest of her life. Please inform us, may Allah reward you with good, about the ruling on the expiation in this case, and how I should feed sixty poor people if the answer is so? Is the expiation obligatory as long as the medical advice recommends not fasting out of fear for her life? Is the expiation obligatory every year? Is it permissible to pay the expiation in cash, and what is the amount? Is it permissible for me to buy grain and divide it, or to go down to the Haram and distribute cash to the poor because there are not sixty people available? Please inform us, may Allah reward you with the best reward.
A: If the trustworthy Muslim doctor advises that fasting will harm her, then she
breaks her fast and pays expiation for every day of Ramadan by feeding a poor person half a sa' of wheat, rice, dates, or similar local food, and it is not permissible to pay the expiation in cash. And success is from Allah, and may Allah send prayers and blessings upon our Prophet Muhammad, his family, and his Companions.
