Q: About forty years ago, my mother gave birth to a boy who was afflicted with a high fever. At the same time, I had a cousin (my father’s sister’s daughter) who was the same age as my older brother and was afflicted with the same illness. My mother squeezed milk from her breast into half a small coffee cup and mixed it with medicine for this illness. She would give a portion of it to my brother to drink and put a portion in his eyes, and she would give the other portion to my cousin to drink and put a portion in her eyes. My mother’s intention was to give the medicine, not to breastfeed; if the intention had been breastfeeding, she would have nursed her from her breast. The medicine was given over a period of three or four days, half a small coffee cup each day, as I mentioned before. My cousin gave birth to a daughter. The question here is: Is my cousin’s daughter permissible for me to marry, or is she not permissible? Please inform us. May Allah reward you with good.
A: For breastfeeding to be a cause of prohibition, it must occur within the first two years of the child’s life, and it must be five breastfeedings or more. The definition of a breastfeeding is that milk from the nursing mother reaches the child by him taking the breast and sucking milk from it; when he leaves it, whether to rest or to move to another breast, it is counted as one breastfeeding, and so on
until five breastfeedings are completed, whether at one time or at different times. If milk is squeezed into a vessel and given to the boy at one time, it is one breastfeeding; if given to him at five separate times, it is five breastfeedings, whether it is mixed with food, drink, or anything else, as long as the nature of the milk remains. This is based on His saying, the Exalted, regarding those who are prohibited:
your [milk] mothers who nursed you, your sisters through nursingوَأُمَّهَاتُكُمُ اللاتِي أَرْضَعْنَكُمْ وَأَخَوَاتُكُمْ مِنَ الرَّضَاعَةِ
[An-Nisa 4:23]
, and based on his saying (peace and blessings be upon him):
Breast-feeding makes unlawful (for marriages) the same things that blood tie make unlawful.يحرم من الرضاع ما يحرم مـن النسب

. Intention is not a condition for breastfeeding. Therefore, if your mother gave your cousin five feedings of her milk, to which she added some medicine, within the first two years, she becomes your sister through breastfeeding, and the aunt of your children, and you become the maternal uncles of her children, both male and female. If the number of feedings is less than five, she is not prohibited, and she remains a stranger to you. And success is from Allah, and may Allah send prayers and blessings upon our Prophet Muhammad, his family, and his Companions.
